Enrollment Information

Requirements for ALL students:

Drug Testing Policy and Consent 

ALL students 7th-12^th are drug tested within the first 30 days of school. Random drug tests are performed once a month for the remainder of the year. 

Corporal Punishment, Internet User Policy, and Handbook Agreements

Handbook can be found on the school website under the Administration tab

LAPTOP AGREEMENT

ALL students 7th-12^th are issued a laptop. This agreement assures the school the student will be responsible for any damage and/or replacement costs or loss of peripherals (i.e. power cords – the most common replacements.)

Please apply as soon as possible to ensure your child will qualify for this program. This file may be returned to Paulla Piercey.

Driving Permit

If you will be driving to school anytime during the school year, please sign this form and also bring a copy of your driver’s license and insurance verification with you.

Information required of ALL students participating in athletics:

Physical Form 

(If you did not participate in the school provided physicals on July 6^th, you will need to get this form signed by your physician.)
